If you’d like to make a request for advice or make an application for equipment, please read and follow the instructions below.

• Please read through the guidelines, objectives and funding criteria on our homepage.
• Please download the application form and complete the document. Make sure to download and save the form to your computer first and then complete it.
• Send back your form along with a supporting letter via email to imelda.callowhill@gmail.com or via post to the address on the form.

NOTE: APPLICATIONS RECEIVED IN JULY AND AUGUST WILL BE PROCESSED IN SEPTEMBER.

It is a requirement that all applications are supported with a letter from the OT or Physiotherapist assigned to the child before we can consider your application.

Please note that we are only able to support children in the Southend, Castle Point, Rochford and Rayleigh district areas.
